Workers' Compensation

Workers' compensation is a state-required benefit system that provides medical and disability payments for employees who suffer an occupational injury or illness in the course and scope of their employment. All BYU employees are covered by this system. Workers’ compensation benefits are the exclusive remedy for covered injuries or illnesses.

Employee Injury Statement Form

Injured employees can complete and take this form to the Student health Center Urgent Care to speed up the process.

(Unless the injury is an emergency, see the Medical Care section of the Handbook first if you are considering obtaining care at Work Med, an ER, or any other location.)

Return to Work form

Employees take this form to healthcare appointments and provide it, completed by the healthcare provider, to their supervisors as soon as practical.
