How do I ensure my team is ready to respond to a workplace injury or incident?
It is always appropriate to contact the BYU EMS at 801-422-2222 for assistance with any workplace incident.

Action 1: Assist
Get the employee the medical assistance they need. Call 911 for any serious or life-threatening injuries. If you need medical assistance for non-life-threatening injuries, call the BYU Dispatch at 801-422-2222. Help to reduce the stressful situation and stay with the employee until a family member or friend can assist.
Action 2: Notify
If the employee's injuries are severe, life-threatening, or required a call to 911, you also must call the Safety Manager at 801-422-4896. After ensuring the employee has been taken care of, all supervisors must immediately fill out the Supervisor’s Report of Workers’ Compensation Injury.
Action 3: Comply
Comply with the treating physician’s recommendations for light duty if given. Forward the physician’s note to the Safety Manager, or email to safety@byu.edu.
Action 1: Inform
Let your supervisor know about the incident immediately.
Action 2: Collaborate
Work with your supervisor to obtain the necessary medical care. Only drive yourself if the injury is minor and you know you can reach the clinic without difficulty. If not, have your supervisor or another co-worker drive you.
Action 3: Follow
Follow the treating physician’s recommendations, including light duty, if given. Give your supervisor a copy of the light duty note from the physician.
Action 1: Notify
Notify the police and file a report.
Action 2: Provide
Provide the other driver with your license’s information, vehicle registration and insurance information located in the glove compartment of the BYU vehicle.
Action 3: Obtain
Obtain the other driver’s license information, phone number, plate number and insurance.
Action 4: Photograph
Take pictures of the accident.
Action 5: Report
Notify your supervisor and fill out the Driver’s Incident Report.
Action 6: Submit
Submit the report to Risk management with the pictures.
Action 7: Repair
Take the vehicle to the BYU Auto shop for repairs or call them at 801-422-5412 if the vehicle needs to be towed.
Action 1: Assist
If needed, assist the individual in obtaining appropriate medical care. For serious, life-threatening injuries or situations, call 911. For medical assistant, contact the BYU dispatch at 801-422-2222.
Action 2: Document
For incidents involving property damage, general liability, EFY, FSY, Auto accidents, other injuries or other incidents, go to incident.byu.edu/forms/ and fill out the correct form. This also includes incidents or injures to non-employee students in classrooms and labs.
Action 3: Receive Help
Call Risk Management at 801-422-5150 for any assistance with forms, claims, or what to do.
