Other Incidents (General Incident) Report
What is a reportable incident?
An incident is an unplanned accident, occurrence, or event. A general incident that does not fall within the other types of incident categories, but which may result in a financial loss to BYU needs to be reported to the Risk Management Department within 24 hours, if feasible, or as soon as possible via the incident.byu.edu website. The following are examples of a possible financial loss that need to be reported:
- An injury to a BYU employee, BYU student, guest, or visitor on Campus,
- Accidental damage to University owned property including vehicles, buildings, structures, and business equipment,
- Accidental damage to property owned by a third party (other than BYU-owned property),
- An injury to participants in any campus sponsored events, i.e., EFY, FSY, Sports Camps, Conferences, etc.
What should I do immediately after the incident?
If the incident does not fall within the other types of incident categories, please visit the incident.byu.edu website, click on the Other Incident icon to complete the General Incident report form.
What information is needed for the report form?
- Date and Time of incident (AM/PM),
- Location of incident ((street address, description of building, area, parking lot, etc.),
- Description of incident. Please provide as many details as possible.
- Description of injury or damage. Please provide as many details as possible.
- Name of involved person,
- If involved person is a minor, please include name of Parent/Guardian and contact information.
- Phone number (cellular, home/work),
- Email address,
- Current address,
- Employee/Student BYU ID No. or NetID
- Witness Name & contact info
- Name of person reporting incident,
- Email address, and
- Phone number.
What happens after the incident is reported on the incident.byu.edu website?
- An incident report is generated and submitted to the BYU Risk Management and Safety’s Incident Office.
- The incident report is reviewed and investigated further by the appropriate department personnel.
- The incident report is retained electronically by BYU Risk Management and Safety’s Incident Office.
