How to Report an Automobile Accident in a BYU-Owned Vehicle

YOU ARE NOT AUTHORIZED TO MAKE ADMISSIONS OF LIABILITY OR PROMISES OF PAYMENT FOR BYU

  1. Notify police and file a report
    • Off campus: 911
  2. Give the following information to the other driver
    • Information on your driver’s license
    • Information on vehicle registration
    • Information on insurance card in glove compartment
  3. Obtain the following information from the other driver Information on driver’s license
    • Phone number
    • License plate number
    • Insurance company, agent, and policy number
  4. Notify your supervisor
  5. Fill out the Driver’s Incident Report
  6. Turn in the report to the Risk Management and Safety department
  7. Bring damaged BYU vehicle and a copy of the Driver's Incident Report to the BYU Auto Shop (2-5412) for an estimate and repairs
