What is a reportable incident or loss?

An incident is an unplanned accident, occurrence, or event. An incident that could result in a financial loss to BYU needs to be reported to the Risk Management Department within 24 hours, if feasible, or as soon as possible via the incident.byu.edu website. The following are examples of a possible financial loss that needs to be reported:

  • An injury to a BYU employee, BYU student, guest or visitor on Campus,
  • Accidental damage to University owned property including vehicles, buildings, structures, and business equipment,
  • An injury to participants in any campus sponsored events, i.e., EFY, FSY, Sports Camps, Conferences, etc.

How do I report an auto accident involving a BYU-owned vehicle?

If the incident involves a BYU-owned vehicle, please visit the incident.byu.edu website, click on the Automobile Accident icon to complete and submit a Driver’s Incident Report form.
What information is needed for the Driver's Incident Report?

  • Date and time of accident (AM/PM)
  • Location of accident (street address, city/state, nearby bldgs., parking lot, landmarks, etc.)
  • Detailed description of accident (sideswipe, head-on, rear-end, backing, broadside, etc.) Please provide as much detail as possible. Include diagram if possible.
  • Photos of the accident location and damage to both vehicles or property
  • Year/Make/Model/Fleet No. of BYU-owned vehicle
  • Current location of BYU-owned vehicle post-accident
  • Names and contact information of passengers in BYU-owned vehicle
  • Detailed description of damage (bumper, fender, hood, roof, trunk, etc.) to BYU-owned vehicle
  • Description of other involved vehicle (Year/Make/Model) or property
  • Insurance information for other involved vehicle and/or driver
  • Detailed description of damage to other involved vehicle or property
  • Contact information of other driver and passengers including name, driver’s license number, phone number, address, email, etc.
  • Contact information of any witnesses including name, phone number, address, email, etc.
  • List any reported injuries to anyone involved in the accident
  • List emergency responders, i.e., EMTs, Ambulance, Fire and Police Departments and case numbers
  • Provide any other additionally important and relevant details

What happens after the incident is reported on the incident.byu.edu website?

  • An incident report is generated and submitted to the BYU Risk Management and Safety’s (BYU RM&S) Incident Office. Upon receipt, the incident is submitted by RM&S’s incident office to The Church of Jesus Christ of Latter-day Saint’s Risk Management Department (CRMD) and a claim is generated through their Global Incident Reporting (GIR) system.
  • Once the claim has been received by CRMD, the auto loss claim is assigned for adjusting to Sedgwick Claims Management Services, Inc. (SCMS), a contracted third-party claims administration (TPA) corporation.
  • A claims adjuster may contact you to obtain a statement and/or other details of the accident.
