Incident/Loss Reporting Guidelines

What is a reportable incident or loss?

An incident is an unplanned accident, occurrence, or event. An incident that could result in a financial loss to BYU needs to be reported to the Risk Management Department within 24 hours, if feasible, or as soon as possible via the incident.byu.edu website. The following are examples of a possible financial loss that need to be reported:

  • An injury to a BYU employee, BYU student, guest or visitor on Campus,
  • Accidental damage to University owned property including vehicles, buildings, structures, and business equipment,
  • Accidental damage to property owned by a third party (other than BYU-owned property),
  • An injury to participants in any campus sponsored events, i.e., EFY, FSY, Sports Camps, Conferences, etc.

How do I report an incident?

***If the incident involves a criminal act or theft, please contact the Incident Management Office or Risk Analyst prior to completing the property loss report at 801.422.5150.***

  • If the incident involves a BYU-owned vehicle, please visit the incident.byu.edu website, click on the Automobile Accident icon to complete and submit a Driver’s Incident Report form.
  • If the incident involves BYU owned buildings, equipment, or business property that has been damaged, please visit the incident.byu.edu website, click on the Property icon to complete and submit a Property-Loss Claim Form.
    • Damages to BYU-owned business property or equipment must be estimated at $800.00 or above for reporting purposes.
    • Damages to BYU-owned real property, (i.e., buildings, structures, etc.) must be estimated at $2,000.00 or above for reporting purposes.
    • Damages to a third party’s or non-BYU-owned property in any amount.
  • If the incident involves an injury to a minor participant in an EFY/FSY event, please visit the incident.byu.edu website, click on the EFY/FSY icon to complete the Injury/Illness Report form.
  • If the incident involves an injury to a BYU employee, visit the incident.byu.edu website, click on the Employee Injury icon to complete the Supervisor’s Report of Workers’ Compensation Injury report form.
  • If the incident involves an injury to a guest, visitor, or BYU student, please visit the incident.byu.edu website, click on the Other Injury icon to complete the Injury Report for Departments form.
  • If the incident does not fall into any other category, please visit the incident.byu.edu website, click on the Other Incident icon to complete the General Incident Report form.

  • If the incident is a general incident, near miss or if there is a safety issue, please visit the incident.byu.edu website, click on the Safety Concern icon to complete the General Incident, Near Miss, and Safety Report form.
