Automobile Accidents

What is the protocol if a BYU employee is involved in an auto accident while operating a BYU owned vehicle?
 The driver needs to complete and submit the Driver’s Incident Report form. Please refer to the form for further information and directions.

Claims

How does my Department make a claim to pay for property losses?
 A department may make a claim by completing and submitting a Property Loss form. Please refer to the form for further information and directions.
 
I submitted a claim to RM&S but I have not heard anything further.
 If you have not received a confirmation and/or follow-up information regarding your claim, please contact Risk Management Incident’s office at 801-422-1203 or 801-422-5779.
 
Does my Department have to replace the lost property before it can be reimbursed?
 If it is a covered loss per insurance policy standards, the department needs to replace the item prior to being reimbursed. Insurance will pay for the replacement cost of like, kind and quality property. Upgrades will not be reimbursed by insurance.
 
Can we submit proof of replacement with a Purchase Order or a Purchase Requisition, Delivery Receipt, or an internet Order Confirmation?
 No. A receipt or invoice is required for reimbursement. If the item is reparable, a final repair invoice is required for reimbursement.
 
How does our Department get paid?
 Payment will be issued to BYU Financial Services and your department will be credited through the PeopleSoft account number that you provide on the claim incident report form.

Injuries

What about injuries?
 For injuries to employees that occur on-the-job, within 24 hours of the injury, please complete and submit the Supervisor’s Incident Investigation Report form. Please refer to the form for further information and directions.

For injuries to students, guests, tenants, and visitors that occur in your Department’s purview, please complete and submit the Injury Report for Departments. Please refer to the website for further information and directions.

Lawsuits

What about lawsuits and civil rights claims etc.?
 All lawsuits and litigated matters are answered through the BYU Office of General Counsel.
