  1. Notify police and file a report Campus:

  2. Give the following information to the other driver
    Information on your driver’s license
    Information on vehicle registration
    Information on insurance card in glove compartment
  3. Obtain the following information from the other driver Information on driver’s license
    Phone number
    License plate number
    Insurance company, agent, and policy number
  4. Notify your supervisor
  5. Fill out the Driver’s Incident Report
  6. Turn in the report to the Risk Management, Safety and Compliance Department

  7. Bring damaged BYU vehicle and a copy of the Driver's Incident Report to the BYU Auto Shop (2-5412) for an estimate and repairs
